 HOMECARE SERVICES

·         (SNV)   SKILLED NURSING VISIT  -  Right at your home, our nursing team will provide the following services:

Complete assessment

bullet

 Based on the assessment, design the care plan with required care services.

Provide information and advices on diabetic management

bullet

Perform cardiopulmonary assessment

bullet

Teach management skills of hypertension.

bullet

 Monitor hypertension

bullet

Draw blood work as ordered by a physician

bullet

Drop off specimens

 

bullet

   (NV)    NURSING VISIT -  For one or two hours minimum, our skilled nurse team will go to your home and prefill medication planners and other related services.

  (RN)     REGISTERED NURSE / LPN)    LICENSED PRACTICAL NURSE) – The team will administer nursing care by the hourly basis program  for long and short term nursing care requirement. 

            (LNA)   Licensed Nurse Assistant  / (C.N.A) Certified Nurses aid -    The team will provide:

Limited catheter care

Blood glucose monitoring

bullet

 Blood pressure monitoring

bullet

Assist with bathing and dressing

bullet

Dressing and changes

bullet

 Meal preparation

bullet

Spot cleaning

 (PCSP)   Personal Care Service -  The team will provide the following:

Assistance with bathing

Assistance with dressing

Light homemaking duties.

 (HMK)  HOMEMAKERS – The team will provide a variety of services such as:

Housekeeping

bullet

Dusting

Vacuuming

Damp moping

bullet

Cleaning bathrooms

Laundry

bullet

Changing bed linen

bullet

 Meal preparation

bullet

Errands and shopping

·         COMPANION  -   The team will provide the following services:

Assuming the well being and the safety of the client

Play Cards
