HOMECARE SERVICES

 

· Skilled Nursing  right at your home, our nursing team will provide the following services:

· A complete Assessment

· Refill medication planner

· Based on the assessment, design the care plan with the required   services

· Provide information and advices on diabetic management and others.

· Perform Cardiopulmonary assessment

· PT INR services

· Teach skills of hypertension

· Supervise the CAN, HHA  & HM

· Home Makers this team will help with housekeeping, grocery shopping and cooking.

· Companion Provide companionship as needed for specific days and time. 

· Home Health Aid  The team will provide  medication reminders, limited catheter care, Blood Glucose monitoring, Assist with bathing and Dressing , Prepare meals and spot cleaning         

· IN HOME DAY CARE   -  The team will provide  assistance throughout the day as requested.

· RESPITE    The team will provide respite care as needed.

· SLEEP IN   The team will provide assistance  overnight  as requested.

· LIVE IN   -   Provide care 24 / 7  as  requested for a specific period of time.

· COMPANION    -  Provide companionship as needed for specific days and time.

· ERRANDS   -  Drop off and pick up medication, pick up groceries, take the client to doctors appointments and other places.

 

OTHER SERVICES

Interpretation and translation:  The team will provide  interpretation and translation to citizens that are not fluently English
